About The Position

Sentara Health is seeking a Senior Community Health Worker to support community outreach, resource coordination, and care navigation for individuals in the community. This role acts as a Team Lead, supporting analytical/clinical work within the ICM CHW program and serving as a subject matter expert for all CHW operations. The Senior CHW collaborates with the ICM Manager to ensure efficient and effective program operations, can assume responsibility for daily CHW operations, attends meetings and training, prepares departmental statistics, and performs other clerical duties. They educate and support employees with CHW processes and workflows, act as an SME for new process rollouts, and perform special work assignments as needed. This role supports the ICM Manager, Director, and other leadership team members, collaborates with cross-departmental teams, and takes initiative to implement new processes independently. The Senior CHW acts as a role model for the CHW team and a subject matter expert in CHW areas.

Requirements

  • High school diploma or equivalent
  • Valid driver's license

Responsibilities
