About The Position

The Clinical Resource Coordinator (CRC) functions as the clinical nurse manager and oversight for assigned Ellis Medical Group Outpatient Practice location(s). They provide support to the Practice Administrator in planning, coordinating, staffing, and supervising nursing care delivery to ensure efficient and effective patient care. The CRC is responsible for compliance with departmental and regulatory standards of care and practice. In partnership with the patient and family, the CRC collaborates with physicians and other inter-disciplinary team members to establish a plan of care with targeted goals and interventions that lead to optimal outcomes for identified at-risk populations. Working in collaboration with the Practice Administrator, Clinical Educator, and other respective team members, the CRC provides care using best practices. By maximizing the patient’s self-care abilities, the CRC uses the nursing process to assess, plan, implement, and evaluate clinical strategies leading to optimal clinical results, optimal cost utilization of resources, and exceptional patient experience. The CRC utilizes clinical assessment skills to maximize patient access to appropriate care at the appropriate time. This role involves problem-solving, coordination of assignments and schedules, and evaluating clinical staff performance. The CRC serves as the clinical resource for the practice.

Requirements

  • Current NYS Registered Nursing license required.
  • Current BLS required.
  • BSN in nursing required.
  • A minimum of 5 years of clinical nursing that includes experience with nursing assessment, patient education, and nursing care plan development.
  • Clinical triage experience required.
  • Leadership experience, 1-2 years in a clinical setting required; ambulatory care setting preferred.
  • Excellent analytical and interpersonal communication skills necessary to correspond with patients, families, providers and third party payers.
  • Demonstrated ability to work collaboratively with physicians and other professionals in a direct and positive manner.
  • Demonstrated ability to lead a clinical team and mentor non-licensed clinical personnel.
  • Demonstrated critical thinking skills and ability to prioritize patient care and tasks.
  • Maintains knowledge of all policies and procedures related to patient care.

Responsibilities

  • Serves as a clinical resource to all clinical team-members by establishing clinical expertise in management of assigned population.
  • Maintains current standards of care for ambulatory nursing.
  • Supports the office clinical staff in coordination of the interdisciplinary team.
  • Supervises daily clinical operations of the clinical staff.
  • Works in collaboration with the operations supervisor and administrator to ensure staffing, workflows and processes are in place to support successful operations.
  • Facilitates regular clinical staff meetings and daily huddles to maintain open lines of communication and awareness of expectations to all clinical team members.
  • Coordinates clinical staff schedules (in collaboration with Practice Manager / Administrator).
  • Acts as liaison for patients, providers and other care team members.
  • Partners with office administrator for the evaluation of clinical staff by completing and/or providing clinical feedback for annual Performance Evaluations as well as probationary evaluations.
  • Assists clinical educator and site administrator in development of all clinical workflows that pertain to the practice.
  • Responds to patients’ clinical concerns and performs service recovery when needed.
  • Performs POC testing and manages POC competencies for clinical staff within the practice.
  • Signs off on clinical point of care competency validations and maintains up-to-date records of competencies.
  • Oversees Point of Care daily/weekly/monthly Quality Control/ Verifying data is transmitted/recorded daily (EKG, logs).
  • Enforces Scope of Practice with clinical staff; ensures clinical staff are working within scope of practice.
  • Assumes responsibility for education and self-development in collaboration with the Clinical Education and Site Administrator; distribution and review of staff education as related to clinical and regulatory topics.
  • Responsible for the maintenance of best practices and Joint Commission standards as they apply to the practice(s) to which the CRC is assigned.
  • Performs weekly JC tracers and initiates corrective action for applicable findings; including hand hygiene audits.
  • Manages sample medications and is responsible for compliance of the sample log and adhering to the sample medication policy.
  • Oversees inventory and ordering of medications, vaccines and VFC; oversees staff education for VFC.
  • Anticipates patient equipment and supply needs and delegates responsibility for ordering and delivery of these items.
  • Promotes and reinforces clinical policy and procedure; escalates as needed to practice administrator.
  • Completes mid-day huddles with providers for pre-visit planning.
  • Works in collaboration with clerical staff for patient scheduling concerns.
  • Able to respond appropriately to clinical emergencies in the office.
  • Delegates and monitors clinical tasks/responsibilities.
  • Ensures all clinical documentation and correspondence is complete in EMR.
  • Rooms patients as needed to maintain efficient flow of practice schedule.
  • Backs up staff with any needs (rooming, injections etc.).
  • Performs other duties as assigned / as required.
  • Partners with identified at-risk patients throughout the diagnosis, treatment and follow up to deliver continuity of care.
  • Anticipates the needs of the patient, recognizes and responds to changes in the patient’s status and determines priorities of patient care.
  • Maintains a patient-centered approach to care; Evidence Based Practice.
  • Identifies actual and potential patient barriers to learning. Reports barriers to responsible provider / care team and collaborates with them to develop alternatives to traditional learning resources.
  • Triage patient symptoms and facilitates the appropriate care.
  • Assesses and manages patients’ post-operative and post-procedure status and relays treatment plan within the RN’s scope of practice.
  • Provides patient education regarding all aspects of their disease process, medications and medical concerns.
  • Monitors pathology and other lab reports; communicates abnormal values to providers.
  • Performs necessary point of care testing.
  • Assumes an active role in the Ambulatory Nursing Quality Committee.
  • Initiates and obtains prior authorizations for medication coverage by governmental and private insurers as needed.
  • Focuses on providing each patient with an exceptional service experience.
  • Responds to clinical pool messages in a timely manner.
  • Handles refill requests sent to provider; includes looking up I-stop for controlled substances.
  • Responds to patient Non-triage phone calls and addresses needs.
  • Completes Physician forms and letters (including obtaining any necessary records).
  • Performs Warfarin/INR monitoring and reporting.
  • Performs patient swabbing/specimen collection.
  • Performs EKG.
  • Performs patient assessment.
  • Conducts Annual Wellness Visits/Medicare physicals.
  • Administers Flu/pneumococcal standing orders.
  • Knowledge of adult and pediatric vaccine schedules.
  • Administers medications per Ellis policy and appropriate medication rights.

  • Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
