About The Position

The Senior Clinical Care Navigator (SCCN) serves as the lead architect for complex patient journeys and multi-system coordination needs within Carrum Health's Cardiology Service Line. This role requires mastering the intersection of clinical excellence and operational strategy, managing high-acuity cases, and refining workflows for care continuity. The SCCN collaborates with clinical, operational, and external partners to ensure elite patient outcomes and shapes care delivery design and execution at scale.

Requirements

  • Registered Nurse with a BSN from an accredited School of Nursing.
  • Minimum of 8+ years of progressive clinical experience in Cardiology.
  • Proven track record of reliability, growth, and increasing scope of responsibility.
  • Technical savvy to collect, report, and present program metrics (via Excel, PowerPoint, etc.) to drive quality improvement and regulatory compliance.
  • Ability to manage complex workflows, high-impact responsibilities, and advanced patient scenarios.
  • Commitment to providing the highest level of service through empathy, professionalism, and integrity.
  • Ability to cut through the noise of a fast-paced start-up environment and affect real change.
  • Appropriate handling of confidential information and adherence to HIPAA requirements.
  • Comfort interacting through a variety of tech platforms and ability to quickly learn new software.
  • Current and unrestricted Registered Nurse licensure.

Nice To Haves

  • Master’s degree in Nursing (MSN) or Healthcare Administration.
  • Spanish Speaking.
  • Board certification in Cardiac-Vascular Nursing (RN-BC) or as a Care Coordination and Transition Management (CCCTM) professional.

Responsibilities

  • Manage a dedicated caseload of cardiology patients; identify new candidates and facilitate all necessary referrals for testing and procedural appointments.
  • Participate in the creation and evaluation of educational materials and clinical care paths to ensure adherence to medical plans and behavioral modifications.
  • Work closely with management on daily operations, including short- and long-range planning to improve service delivery and patient satisfaction.
  • Serve as a subject matter expert and resource for the care team, providing focused education on cardiac diagnosis management and quality of life matters.
  • Own the end-to-end success of your service line by ensuring patients experience clear, coordinated, and timely care throughout their journey.
  • Manage our most complex care coordination and navigation scenarios, ensuring patients move seamlessly through every stage of their journey.
  • Coach, mentor, and support junior team members by reinforcing best practices and explaining the “why” behind workflows and decisions.
  • Identify operational gaps and opportunities for improvement, contributing insights that enhance the overall patient experience.
  • Act as a Change Champion, providing stability and leadership as processes, tools, and organizational needs evolve.
  • Collaborate cross-functionally to support workflow improvements and ensure alignment with patient experience goals.
  • Serve as a trusted connector between clinical experts within our Centers of Excellence and clinical operations teams, ensuring workflows reflect both clinical intent and real-world execution.
