The Cardiology Care Navigator plays a crucial role in coordinating the patient journey from referral through the completion of diagnostic testing and follow-up care. This position serves as the primary point of contact for non-clinical patient coordination, ensuring patients are scheduled for recommended imaging and follow-up appointments before leaving the clinic whenever possible. The navigator monitors patients requiring various cardiovascular diagnostic procedures, tracks their progress through the imaging process, and coordinates with clinical staff to ensure all necessary preparations and instructions are provided. Additionally, this role is involved in care gap management, program growth initiatives, and reporting on key performance indicators to clinic leadership.
