About The Position

Our senior care coordinators are your advocates and can connect you to short- and long-term care resources for your unique needs. They provide personalized support and guidance to navigate through the healthcare system and assist with scheduling and coordinating transportation for appointments. In addition, they are skilled in evaluating everything from what home safety equipment might be helpful to addressing emotional concerns with you. A senior care coordinator specializes in working with seniors on care decisions — from whom to see for care to helping coordinate and manage care needs and services. They help clients create a plan for care now and later, and identify the places and professionals who can deliver that care. They also ensure necessary communication among family members and the care team to keep everyone updated and on the same page.

Requirements

  • Bachelor’s Degree in Nursing (BSN), Social Work (MSW), Healthcare Administration, or Gerontology.
  • Active New Jersey license (e.g., Registered Nurse [RN] or Licensed Practical Nurse [LPN]) depending on the medical complexity of the seniors being served.
  • 3 to 5 years of professional care coordination or case management experience.
  • 1-2 years specifically in aging services or home care.
  • Ability to assess patient needs, create proactive care plans, and facilitate transitions of care between independent living, assisted care, and hospitals.
  • Fluency in New Jersey Department of Health (DOH) regulations, Medicaid/Medicare guidelines, and long-term care quality standards.
  • Proficiency with Electronic Health Records (EHR) systems and Microsoft Office.
