Requirements
- Strong communicator, both written and verbal, who easily connects with people.
- Ability to assess needs and create action plans that drive real results.
- Experience working with multiple population types and advocating for individuals.
- Problem-solving and conflict resolution skills.
- Ability to work as a team player and independently.
- Approach cultural and economic differences with sensitivity and understanding.
- Minimum of two (2) - four (4) years of education and/or experience in a related field required.
Nice To Haves
- A four-year post-secondary degree in Human Services is preferred.
Responsibilities
- Provide case management services that help individuals and families set and achieve personal goals.
- Conduct assessments, develop individualized plans, and assist in stabilizing housing situations.
- Keep detailed records, document all contacts, and maintain participant confidentiality.
- Support participants in becoming independent from public assistance.
- Build relationships with service providers, attend networking meetings, and advocate for participants.
- Ensure accurate documentation, track referrals, and complete necessary reports on time.
