Secondary Case Manager
About The Position
Well Care Services is a Behavioral Health Clinic looking for Case Managers to join our team. We are hiring a team of case managers that will be working hands on with patients out on the field. The role of a secondary case manager is to assist primary case management with patient monitoring and management of their treatment plan. Secondary CM's are expected to assist with maintaining oversight of the members in the housing program with verifying treatment compliance, medication management, and client intervention if needed. They will also assist with general coordination of care.

- Must be able to pass a drug and background check.
Responsibilities
- Assist with maintaining oversight of members in the housing program.
- Verify treatment compliance.
- Assist with medication management.
- Provide client intervention if needed.
- Assist with general coordination of care.
- Coordinate with the pharmacy on patient medications.
- Document interventions in EHR.
- Coordinate telehealth appointments.
- Coordinate clinical appointments.
- Monitor patients for personal hygiene, room cleanliness, and sobriety.
