Requirements
- Active Physical Therapist license in the state of Michigan
- Proven experience in homecare physical therapy
- Excellent interpersonal and communication skills
- Strong organizational skills and attention to detail
- Reliable transportation and ability to travel to patient homes in the Southeast MI Region.

Responsibilities
- Provide skilled physical therapy services in patients' homes according to established care plans
- Evaluate patient progress and adjust treatment plans as necessary
- Communicate effectively with patients, families, and healthcare team members
- Maintain accurate documentation and comply with company policies and regulatory requirements
- Demonstrate professionalism and empathy while fostering patient independence
