About The Position

The City Connects School Site Coordinator is responsible to both the Principal and the City Connects Supervisor. This role involves leading/facilitating a Student Support team (STAT) to review intensive student needs and create actionable plans. The coordinator will also facilitate yearly discussions with classroom teachers to assess student strengths and needs across developmental domains, leading to individualized student support plans. The position focuses on connecting students to prevention, early intervention, and intensive services, developing community partnerships, collaborating with school staff to implement the City Connects model, engaging families, collecting data, and participating in professional development. Requirements

• Master’s degree in School Counseling, Mental Health Counseling, Social Work or other related field (ex. Psychology).
• Evidence of culturally competent practices.
• Ability to work collaboratively with a diverse staff, a diverse student body and families, as well as a team of professionals at both the school and the university.

Nice To Haves

• Experience working in a school environment is strongly preferred.
• Experience in urban public schools.
• Fluency in a second language.
• Experience with an organization that uses data to guide practice.
• Licensure as a School Counselor, Licensed Mental Health Counselor (LMHC), or Licensed Independent Clinical Social Worker (LCSW / LICSW).

Responsibilities

• Lead/facilitate a Student Support team (STAT) to review the intensive needs of referred students and create an actionable plan.
• Facilitate a yearly discussion with each classroom teacher to assess the holistic strengths and needs of each student across developmental domains (academics, social/emotional/behavioral development, physical health, and family).
• Develop individualized student support plans for each student.
• Connect students to prevention, early intervention, and intensive services programs that address physical, emotional, cognitive, and social development.
• Develop and enhance community partnerships with varied community agencies to meet identified needs of students, families, and the school.
• Collaborate with school staff to implement the City Connects model.
• Engage families and caregivers in understanding and supporting their children's education and development.
• Collect data on the implementation of the model and the delivery of services.
• Participate in professional development sessions to implement the City Connects model.
• Pursue continuous professional growth.
• Schedule and present information about student support services, roles, responsibilities, and the referral process to school staff and administrators by October 1st annually.
• Perform other related duties as assigned.
