About The Position

The School-Based Health Coordinator (SBHC) serves as a vital bridge between the K–12 education system and healthcare partners to improve both academic success and health outcomes for students and families. The SBHC acts as a trusted resource and point of contact, ensuring access to integrated health services within school environments while building strong, collaborative relationships with school staff, families, and community partners. The SBHC coordinates school-based health initiatives across assigned schools, supporting student health, wellness, and access to care. Responsibilities include facilitating healthcare services, reducing barriers to care, supporting family engagement, and aligning services with student academic and social-emotional needs. The role also includes program tracking, reporting, and communication to support informed decision-making and continuous improvement.

Nice To Haves

  • State Certified Translator Preferred.
  • School-Based Health Coordinator I: 1–2 years of experience in healthcare, education, public health, or social services preferred
  • Experience working with children and families
  • Ability to support coordination of programs under guidance
  • School-Based Health Coordinator II: 3–5 years of experience in healthcare, education, public health, or social services
  • Experience managing multiple programs, sites, or partnerships
  • Strong ability to work independently and exercise sound judgment

Responsibilities

  • Coordinate School-Based Health Services: Facilitate access to mobile clinics, telehealth services, and in-person care. Coordinate logistics including scheduling, site readiness, and partner communication across assigned schools.
  • Health Needs Assessment and Prioritization: Assess and prioritize student health needs using Tier 1, 2, and 3 frameworks in collaboration with school and healthcare partners. Align services with academic and social-emotional needs.
  • Community Collaboration and Family Engagement: Coordinate and participate in family engagement activities such as workshops, school events, and health fairs. Support culturally responsive outreach to engage diverse families.
  • Collaboration with School Staff: Partner with administrators, teachers, counselors, and school nurses to identify and support students facing health and access barriers.
  • Caseload Management and Student Advocacy: Maintain a caseload of students, ensuring consistent follow-up and responsiveness to urgent needs. Advocate for equitable access to healthcare services.
  • Health Clinic and Event Coordination: Plan and coordinate school-based clinics and events, including screenings, immunizations, and physicals. Ensure alignment with school requirements.
  • Communication and Coordination: Maintain clear communication with students, families, school staff, and providers regarding services, referrals, and care coordination.
  • Tracking, Reporting, and Data Management: Document services and track key metrics. Prepare reports on service utilization, outcomes, and program effectiveness.
  • Leadership Communication and Updates: Provide updates to leadership regarding program activities, outcomes, and operational needs.
  • Stakeholder Collaboration: Meet regularly with school and district leadership to align health services with district priorities.
  • Professional Development: Participate in ongoing training and development to maintain knowledge of best practices and compliance requirements.
  • Preventative Health Programming: Support implementation of health education and preventative programs across school sites.
  • Family and Student Development: Coordinate services and programs that strengthen family engagement, student wellness, and academic success.
  • Perform other duties as assigned
