About The Position

ConnextCare is seeking an experienced therapist with credentials as a Licensed Master Social Worker (LMSW) or Licensed Mental Health Counselor (LMHC) to join its Outpatient Mental Health team. This role will be based within the APW Elementary School Based Health Center. The LMSW/LMHC will collaborate with other mental health providers, medical staff, nursing staff, and the management team to ensure the highest standard of clinical quality. The position involves working with school-aged children (grades K-12) and their caregivers who are experiencing emotional and behavioral concerns. The ability to refer severe cases to appropriate community or medical agencies is required. The individual must be capable of managing a full patient load, including all necessary documentation, treatment planning, and scheduling. Other duties and responsibilities as requested by management and supervision will also be part of the role. Clinical supervision is provided. Experience with electronic health records (EHR) and electronic medical records (EMR) is strongly preferred.

Requirements

Credentials as Licensed Master Social Worker (LMSW) or Licensed Mental Health Counselor (LMHC).
Ability to carry full patient load, including all necessary documentation, treatment planning, and scheduling.
Experience with school-aged children grades (K-12) and caregivers with emotional and behavioral concerns.
Ability to refer severe cases to appropriate community or medical agencies.
