About The Position
ConnextCare is seeking an experienced therapist with credentials as a Licensed Master Social Worker (LMSW) or Licensed Mental Health Counselor (LMHC) to join its Outpatient Mental Health team. This role will be based within the Pulaski Jr/Sr School Based Health Center, serving school-aged children (K-12) and their caregivers. The counselor will collaborate with a multidisciplinary team, manage a full patient caseload including documentation and treatment planning, and refer severe cases to appropriate agencies. Clinical supervision is provided.
Requirements
- Credentials as a Licensed Master Social Worker (LMSW) or Licensed Mental Health Counselor (LMHC).
- Experience working with school-aged children (K-12) and caregivers.
- Ability to carry a full patient load, including documentation, treatment planning, and scheduling.
Nice To Haves
- Experience with electronic health records (EHR) and electronic medical records (EMR).
Responsibilities
- Collaborate with mental health providers, medical staff, nursing staff, and management team to ensure the highest standard of clinical quality.
- Work with school-aged children (grades K-12) and caregivers addressing emotional and behavioral concerns.
- Refer severe cases to appropriate community or medical agencies.
- Carry a full patient load, including all necessary documentation, treatment planning, and scheduling.
- Complete other duties and responsibilities as requested by management and supervision.
