About The Position

Community Care Network is seeking a School Based Clinician in Rutland, Vermont. Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human services. The School Based Clinician is responsible for the provision of diagnosis and evaluation, psychotherapy, community supports, service planning, and coordination to all children who are needing additional mental health supports. Those services can include crisis interventions, trauma informed supports and resources, which are embedded directly into the child’s school and therefore more easily accessed. The services provided will help to improve social skills, emotional regulation, problem solving and the overall mental health service goals for the child. The clinician will develop and maintain close working relationships with internal and external professional staff to provide coordinated, quality services and wrap around supports.

Requirements

  • Master's Degree in Psychology, Counseling or Social Work required.
  • 2-3 years' experience providing mental health services for diverse range of school-aged children living with serious emotional disorders required.
  • Proficient with Microsoft Word and Outlook required.
  • Proficient using computer and necessary software as well as experience with navigating an Electronic Medical Record to complete client documentation.
  • General knowledge of community resources, standard models of service coordination, and/or experience working in a variety of human services settings.
  • Structured and organized to support those in crisis and in need.
  • Makes a positive first impression with clients, team members and outside agencies and presents a professional image - as reflected in the employees' work attire and demeanor.
  • Maintains positive team relations with all staff, customers and clients.

Responsibilities

  • Provide clinical assessments and interventions for students and families.
  • Maintain case rate expectations which for the school year (PBIS school) consists of 9 Medicaid eligible students being seen for support/services for 2 hours per month. For a (non PBIS school) those expectations are 10 Medicaid eligible students being seen for support/services for 2 hours per month.
  • Provide individual, group and family therapy, based on emotional need, maintaining a full case load, based on mental health standards and ethics.
  • Provide follow-up and implementation of reintegration plans for clients who are returning to school from hospitalizations and/or facility stays; and comprehensive follow up to all clients served.
  • Enhance awareness of mental health needs, services, and to promote community understanding and response to mental health problems.
  • Make home visits to support families.
  • Facilitate communication between agencies on behalf of the client.
  • Attend team meetings to plan and monitor services.
  • Provide case management, collateral contact and social support, as needed.
  • Assists family to locate and appropriately use community resources, agencies, organizations and team members to coordinate services and monitor progress toward treatment goals.
  • Provides consultation services to teachers, community partners, and school personnel, as needed. Consultation can be educational or evaluative.
  • Also provides consultation and support to staff.
  • Completes and submits all documentation in accordance with specific deadlines, including but not limited to progress notes, assessments, discharges and service referrals.
  • Completes and submits unusual incident reports and follow-ups as required by supervisor.
  • Ensures optimal communication by relaying pertinent client care information to the supervisor.
  • Ensure completion of required follow up visits with client in the community as needed, which includes transportation.
  • Appropriately interprets and applies agency policies and procedures to job responsibilities.
  • Maintain adherence to ethical and legal standards of practice.
  • Maintain client confidentiality, always adhering to all federal and state laws and regulations.
