About The Position

The Patient Service Coordinator (PSC) supports home health operations in a non-clinical capacity, serving as a liaison between patients, caregivers, clinicians, and office personnel. The role coordinates services, manages administrative processes, supports scheduling activities, and helps ensure accurate, timely communication for quality patient care and operational efficiency. The position is on-site in Hawthorne, NY, Monday through Friday, 8:00 A.M. to 5:00 P.M.

Requirements

High school diploma or GED equivalent.

Nice To Haves

At least 1 year of scheduling experience in a healthcare environment.
At least 1 year of administrative experience in a healthcare environment.

Responsibilities

Ensure patient assessment visits, including all OASIS visits, are scheduled and performed on time.
Assign and schedule clinicians using efficient geography, appropriate discipline and skills, and available clinician capacity.
Collaborate with field clinicians to ensure patient visits are scheduled and completed as ordered and within the care center.
Manage calls from patients and field staff regarding scheduling issues.
Refer clinical and performance-related issues to the clinical manager.
Monitor hospitalized patients, communicate with the team, and ensure appropriate follow-up.
Support the clinical manager and coordinate timely communication with patients, clinicians, referral sources, the business development team, and other office personnel.
Run applicable reports and take appropriate action in response to workflow needs.
Assist with internal or external patient transfers between care centers and/or hospice services.
As needed, maintain the supply closet, check that supplies are unexpired and packaged appropriately, and liaise with field staff regarding patient supply needs.
Perform other duties as assigned.
