About The Position

Enrich the lives of older adults through community, choice, and continuing care in the Home Health & Hospice Scheduler role. This role is responsible for scheduling home health and hospice staff according to internal staffing models, physician orders, and in compliance with Medicare rules. This position also plays a leading role with regulatory compliance, reporting, and auditing. This role is 100% on-site at our corporate office. Find your sense of belonging at Wesley! You will provide continuing care through these responsibilities Essential functions of this position include the following: On-site presence is required due to daily in-person collaboration with staff and leadership. Works within the EMR and/or HRIS system to create daily, weekly and monthly schedules of patient care visits for all clinical staff according to signed physician orders and in compliance for timeliness according to Medicare rules. Balance staff caseloads. Monitors that visits are scheduled as ordered by reviewing all physician orders. Alerts staff and management when regulatory timeframes approach, such as end of episode date and date of transfer. Ensure customer service satisfaction through follow-up with clients and employees. Scheduling staff with best fit to match to insure client and staff satisfaction. Communicates with the clinical team to coordinate care, facilitating quality care and satisfaction for both staff and patients/families. This position has a key role in establishing effective communication between the care team and patients/families and other healthcare providers. Monitors all clinician Productivity Points and ensure they are being set up for the week to a minimum threshold per job duty to accomplish productivity standards. Monitors submittals of Plans of Care, Orders, and Discharge/Transfer Summaries to the physician. Monitors recertification and Resumptions of care to ensure they occur in compliance with regulatory standards. Monitors Therapy Reassessments, LPN and Aide Supervisory visits and ensure completion in accordance with Medicare Regulations and State WACs. Monitors HUV visits and Symptom Follow up Visits for hospice to ensure they are completed in accordance with Medicare regulations. Be able to view the entire schedule as a whole, be able to think critically and make adjustments to accommodate Medicare requirements. Completes training on EMR and devices (iPads) for both clinical and administrative staff. Can create and utilize tools to follow requirements and ensure compliance. Generate orientation packets and agenda for clinical and administrative staff. Provides back-up to other administrative functions (Ring of Defense) Perform other duties and special projects as assigned.

Requirements

  • Employees are required to be physically present in the office during scheduled work hours as this role requires physical access to on-site systems, records, equipment or personnel.
  • High School diploma, GED or equivalent work experience.
  • Knowledge of community resources, Medicare and state regulations and reimbursement criteria for third-party and private payers.
  • Ability to read, write and communicate proficiently in English.
  • Excellent communication skills (verbal and written), interpersonal skills, and documentation skills.
  • Proficiency with Microsoft Office and other workplace technology including cloud-based software and mobile apps.
  • Self-motivated, self-directed, excellent organizational skills, and ability to effectively manage stress.
  • Critical thinking skills and ability to adapt to changing priorities are a must.
  • Ability to work independently and with minimal supervision.
