About The Position

The Rural Health Transformation Program (RHTP) is a statewide program administered by the NC Department of Health and Human Services (DHHS), funded by the US Centers for Medicare & Medicaid Services (CMS), aimed at improving access and outcomes for citizens in rural North Carolina. The ROOTS Hub Initiative 5 Lead reports to the ROOTS Hub Initiatives Manager and is responsible for supporting the implementation and operational oversight of Initiative 5 within DHHS' defined region 3. This initiative focuses on advancing value-based payment readiness, healthcare sustainability, and innovative financing models for rural primary care providers and health systems. The role involves collaboration with healthcare providers, payers, finance teams, implementation partners, and community stakeholders to drive operational transformation, value-based care readiness, fiscal sustainability, and data-informed healthcare delivery improvements. The Lead serves as a strategic implementation partner, supporting operational planning, stakeholder engagement, reporting, governance, community engagement, and implementation monitoring. Key responsibilities include close collaboration with the ROOTS Hub Initiatives Manager and coordination with region 3 lead anchors to tailor NCRHTP initiatives to meet regional needs and achieve performance objectives such as building rural community hubs, creating models for expanded primary care and chronic disease management, building a robust workforce, and ensuring fiscal sustainability through innovative models. The position requires high emotional intelligence, discretion, independent judgment, strong communication skills, project management, performance improvement skills, and experience with large-scale implementation plans and budgets.

Requirements

  • Bachelor’s degree in public health, healthcare administration, business administration, finance, or related field required.
  • Minimum of two (2) years of experience project managing large and complex engagements.
  • Minimum of two (2) years of experience coordinating across different types of stakeholders.
  • Minimum of two (2) years of experience in an organization or field that demonstrates an understanding of the healthcare systems, Medicaid and/or Medicare payer systems, clinical and social needs on communities’ and/or populations’ access to health services.
  • Knowledge of value-based care, healthcare financing, rural healthcare systems, and population health principles.
  • Strong project management and operational planning skills.
  • Ability to coordinate across healthcare systems and stakeholder groups.
  • Strong analytical, facilitation, and communication skills.
  • Ability to interpret operational and performance data.
  • Ability to manage multiple priorities and deadlines.
  • Proficiency in Microsoft Office applications.

Nice To Haves

  • Master’s degree preferred.
  • Knowledge of Medicaid, Medicare, and healthcare reimbursement structures preferred.
  • Minimum of two (2) years of experience serving or working on behalf of Medicaid and/or Medicare beneficiaries.
  • Population health experience and experience with physical health, behavioral health, I/DD and TBI populations preferred.
  • Certifications in use of project management methodologies preferred.
  • Certification in quality and process improvement preferred.

Responsibilities

  • Lead day-to-day implementation activities related to Initiative 5 priorities, action plans, and deliverables.
  • Support rural provider readiness for value-based care and alternative payment models.
  • Coordinate sustainability planning and operational transformation activities.
  • Monitor initiative milestones, deliverables, and operational performance.
  • Coordinate implementation activities among healthcare, behavioral health, and social service partners.
  • Identify implementation barriers and recommend operational improvements.
  • Establish ROOTS Hub that connects medical, behavioral health, and social services to improve availability and access to healthcare in rural communities.
  • Coordinate within the organization and with lead anchor partners to ensure all Initiative 5 RHTP responsibilities and requirements are met.
  • Supporting the ROOTS Hub Initiatives Manager to track Vaya’s RHTP plan’s compliance and performance against program-related deadlines and milestones.
  • Support development of program-related reports as outlined by Requirements from NC DHHS.
  • Support region 3 anchors and RHTP subrecipients, implementation partners, and community-based organizations in understanding contracting, invoicing, documentation, reimbursement, reporting, and compliance expectations.
  • Build and maintain collaborative relationships with healthcare providers, public health agencies, community organizations, and implementation partners.
  • Facilitate meetings, trainings, and technical assistance activities related to value-based care transformation.
  • Support regional strategy development and implementation planning.
  • Support incorporation of community feedback and regional health priorities into implementation activities.
  • Facilitate collaboration among providers, community organizations, and regional partners.
  • Participate in community engagement activities and ensure community voice is incorporated into implementation planning.
  • Partner closely with project team, finance department, lead anchors and community stakeholders to ensure the milestones and deliverables for RHTP.
  • Foster relationships with rural communities and disproportionately burdened populations.
  • Supporting project plan and providing regular updates to ROOTS Hub Initiatives Manager regarding RHTP successes and challenges.
  • Attend and/or lead RHTP Initiative 5 related meetings.
  • Track implementation progress, performance measures, and operational outcomes.
  • Support preparation of reports, presentations, and implementation updates.
  • Identify implementation risks and recommend improvement strategies.
  • Support compliance with NCDHHS reporting and operational requirements.
  • Collaborate with other Initiative Leads and ROOTS Hub staff to ensure alignment across initiatives.
  • Participate in Hub governance and planning activities.
  • Serve as Initiative 5 subject matter expert as requested.
