The Rural Health Transformation Program (RHTP) is a statewide program administered by the NC Department of Health and Human Services (DHHS), funded by the US Centers for Medicare & Medicaid Services (CMS), aimed at improving access and outcomes for citizens in rural North Carolina. The ROOTS Hub Initiative 5 Lead reports to the ROOTS Hub Initiatives Manager and is responsible for supporting the implementation and operational oversight of Initiative 5 within DHHS' defined region 3. This initiative focuses on advancing value-based payment readiness, healthcare sustainability, and innovative financing models for rural primary care providers and health systems. The role involves collaboration with healthcare providers, payers, finance teams, implementation partners, and community stakeholders to drive operational transformation, value-based care readiness, fiscal sustainability, and data-informed healthcare delivery improvements. The Lead serves as a strategic implementation partner, supporting operational planning, stakeholder engagement, reporting, governance, community engagement, and implementation monitoring. Key responsibilities include close collaboration with the ROOTS Hub Initiatives Manager and coordination with region 3 lead anchors to tailor NCRHTP initiatives to meet regional needs and achieve performance objectives such as building rural community hubs, creating models for expanded primary care and chronic disease management, building a robust workforce, and ensuring fiscal sustainability through innovative models. The position requires high emotional intelligence, discretion, independent judgment, strong communication skills, project management, performance improvement skills, and experience with large-scale implementation plans and budgets.
