About The Position

This position involves assisting the Director of Nursing Services in directing the day-to-day functions of nursing activities within a long-term care facility. The role requires participation in developing and maintaining policies and procedures, ensuring the availability of current reference materials, and overseeing compliance with nursing service procedures. The position also involves administrative duties, committee participation, personnel management, direct nursing care, staff development, safety and sanitation oversight, equipment and supply management, and care plan and assessment functions. The RN is responsible for the overall supervision and management of the nursing staff during their shift, with delegated administrative authority, responsibility, and accountability.

Requirements

Must possess, as a minimum, a Nursing Degree from an accredited college or university.
Must have training in rehabilitative and restorative nursing practices.
2 years or more experience, preferably in a long-term care facility.
Must be able to read, write, speak, and understand the English language.
Ability to read technical procedures.
Ability to read and comprehend policy and procedure manuals.
Ability to effectively present information and respond to questions from managers and employees.
Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Ability to make independent decisions when circumstances warrant.
Ability to solve practical problems.
Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
Leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary for quality care.
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.

Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to nursing care facilities.

Must be willing to seek out new methods and principles and incorporate them into existing nursing practices.
Must be able to relate information concerning a resident’s condition.
Must not pose a direct threat to the health or safety of other individuals in the workplace.
Must possess a current, unencumbered, active license to practice as an RN in this state.
Current CPR certification.
Must maintain all required continuing education/licensing.
Must remain in good standing with the State Board of Nursing at all times.

Responsibilities

Assist the Director of Nursing Services in directing the day-to-day functions of the nursing activities.
Participate in developing, maintaining, and updating written policies and procedures for the nursing service department.
Ensure that reference materials at nurses’ stations are current and recommend new materials as needed.
Ensure that the Nursing Service Procedures Manual is current and reflects facility procedures.
Ensure that all nursing service personnel comply with the procedures set forth in the Nursing Service Procedures Manual.
Make written and oral reports/recommendations to the Director concerning the operation of the nursing service department.
Periodically review the department’s policies, procedure manuals, and job descriptions, making recommendations for revisions.
Assist in developing methods for coordinating nursing services with other resident services.
Ensure that all nursing service personnel are in compliance with their respective job descriptions.
Participate in the development, maintenance, and implementation of the facility’s quality assurance program for the nursing service department.
Participate in facility surveys (inspections) as requested.
Periodically review and participate in updating the resident’s written discharge plan.
Assist the Director in planning the nursing services portion of the resident’s discharge plan.
Meet with nursing staff and support personnel to plan department services, programs, and activities.
Admit, transfer, and discharge residents as required.
Complete accident/incident reports as necessary.
Interpret department policies and procedures to personnel, residents, visitors, and government agencies.
Perform administrative duties such as completing medical forms, reports, evaluations, studies, and charting.
Ensure the confidentiality of assigned user ID code and password for accessing resident/facility information and report any violations.
Ensure the confidentiality of resident’s protected health information and report any violations.
Report any known or suspected unauthorized attempt to access facility’s information system.
Attend various committee meetings of the facility (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment).
Assist in preparing reports of the nursing service programs and activities for committee submission.
Provide information to the Quality Assurance and Assessment Committee as requested.
Participate in developing the agenda for Care Plan Committee meetings.
Participate in regularly scheduled reviews of discharge plans and make them available to the Care Plan Committee.
Implement recommendations from established committees as instructed by the Director.
Ensure a sufficient number of licensed practical and/or registered nurses are available for the tour of duty.
Ensure a sufficient number of certified nursing assistants are available for the tour of duty.
Develop work assignments and/or assist in completing tasks.
Delegate administrative authority, responsibility, and accountability to each Charge Nurse.
Ensure that all nursing assistants are enrolled in or have graduated from an approved nursing assistant training program.
Ensure that all nurse aide trainees are under the direct supervision of a licensed nurse.
Make daily rounds to ensure personnel are performing work assignments according to acceptable nursing standards and report findings.
Monitor absenteeism to ensure adequate staffing and report problem areas.
Ensure that established policies and procedures, including dress codes, are followed by departmental personnel.
Review complaints and grievances from department personnel and make appropriate reports.
Develop and maintain good working rapport with inter-departmental personnel.
Create and maintain an atmosphere of warmth, personal interest, and positive emphasis.
Meet with shift personnel to identify and correct problem areas and improve services.
Review and evaluate the department’s workforce and make recommendations to the Director.
Participate in employee performance evaluations, determine staffing requirements, and make recommendations regarding dismissals/transfers.
Ensure that departmental disciplinary action is administered fairly.
Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
Report known or suspected incidents of fraud.
Ensure that departmental computer workstations are properly logged off or password protected when unattended.
Inform nursing service personnel of new admissions, their expected time of arrival, and room assignment.
Ensure that rooms are ready for new admissions.
Greet newly admitted residents and escort them to their rooms.
Make rounds with physicians as necessary.
Encourage physicians to record and sign progress notes and orders timely.
Inform the Director when physician visits are not made timely.
Consult with the resident, family, and physician in planning resident care.
Notify the resident’s attending physician and next-of-kin of changes in the resident’s condition.
Visit residents daily to observe and evaluate their physical and emotional status.
Ensure direct nursing care is provided by qualified personnel.
Review nurses’ notes for informativeness, descriptiveness, and reflection of resident response to care.
Provide direct nursing care as necessary.
Start IVs and obtain specimens for lab tests as ordered.
Take TPRs and blood pressures as necessary.
Admit, transfer, and discharge residents as necessary.
Report problem areas to the Director and assist in developing corrective action.
Keep the Director informed of resident status and related matters through reports.
Assist the Charge Nurse in monitoring seriously ill residents.
Authorize the use of restraints when necessary and in accordance with facility policies.
Ensure RNs and LPNs comply with medication and supply administration, storage, and control procedures.
Monitor medication passes and treatment schedules.
Review medication cards for completeness, accuracy, and adherence to stop order policies.
Inspect nursing service treatment areas daily for cleanliness and safety.
Ensure residents unable to call for help are checked frequently.
Meet with residents and/or family members as necessary and report problems to the Director.
Assist in arranging transportation for discharged residents.
Inform family members of the death of a resident.
Call funeral homes when requested by the family.
Participate in developing, planning, conducting, and scheduling in-service training classes.
Assist the In-service Director/Educator in developing annual facility in-service training programs.
Assist the Director in planning clinical supervision for nurse aide trainees.
Attend and participate in continuing education programs.
Assist the Safety Officer in developing safety standards for the nursing service department.
Assist the Director and/or Infection Control Coordinator in identifying, evaluating, and classifying tasks with potential exposure to blood/body fluids.
Ensure personnel performing tasks with potential exposure to blood/body fluids participate in appropriate training.
Ensure an adequate supply of personal protective equipment is available.
Ensure personnel follow established procedures for the use and disposal of personal protective equipment.
Monitor personnel to ensure they follow established safety regulations.
Ensure all nursing service personnel comply with established departmental policies and procedures.
Ensure nursing service work areas are maintained in a clean and sanitary manner.
Ensure all resident care rooms and treatment areas are maintained in a clean, safe, and sanitary manner.
Ensure nursing service personnel follow established handwashing and hand hygiene procedures.
Ensure nursing service personnel participate in and conduct fire safety and disaster preparedness drills.
Participate in the development, implementation, and maintenance of the infection control program.
Ensure nursing service personnel follow established infection control procedures for isolation precautions.
Participate in the development, implementation, and maintenance of procedures for reporting hazardous conditions or equipment.
Ensure personnel wear and/or use safety equipment when lifting or moving residents.
Report missing/illegible labels and MSDSs.
Recommend equipment and supply needs of the department to the Director.
Ensure an adequate stock level of medications, medical supplies, and equipment is maintained.
Assist the Maintenance Director in developing a preventive maintenance program for equipment and supplies.
Participate in the development and implementation of procedures for the safe operation of all nursing service equipment.
Ensure only trained and authorized personnel operate nursing service equipment.
Ensure all personnel operate nursing service equipment in a safe manner.
Monitor nursing procedures to ensure efficient use of nursing service supplies.
Ensure appropriate MSDSs are on file for hazardous chemicals.
Participate in the development of written preliminary and comprehensive assessments of resident nursing needs.
Participate in the development of a written plan of care for each resident.
Encourage resident participation in the development and review of their care plan.
Review resident care plans for appropriate goals, problems, approaches, and revisions.
Ensure all personnel involved in resident care are aware of the resident’s care plan.
Ensure nursing personnel refer to the resident’s care plan prior to administering daily care.
Review nurses’ notes to determine if the care plan is being followed.
Assist the Resident Assessment/Care Plan Coordinator with MDS, RAPs, and Triggers.
Review resident’s medical and nursing treatments to ensure they align with the care plan.
Keep abreast of economic conditions and recommend adjustments to nursing services.
Report suspected or known incidence of fraud relative to billings, cost reports, kickbacks, etc.
Assist with the overall supervision and management of the nursing staff.
Perform other duties as assigned.
