About The Position

The Wound Care RN conducts health assessments focused on wound, ostomy, and continence-related care. This role involves performing comprehensive patient evaluations, developing individualized care plans, recommending treatments, educating patients and staff, and collaborating with interdisciplinary teams to improve patient outcomes.

Requirements

  • Minimum 2 years of Medical-Surgical (Med/Surg) nursing experience.
  • Wound Care experience.

  • One of the following current wound care certifications: Certified Wound Ostomy Continence Nurse (CWOCN), Certified Wound Ostomy Nurse (CWON), Certified Wound Care Nurse (CWCN), Certified Wound Specialist (CWS), Wound Care Certified (WCC)

Responsibilities

  • Conduct wound, ostomy, and continence assessments and develop individualized care plans.
  • Assess and document patient responses to wound care treatments.
  • Evaluate the quality and effectiveness of wound care services provided by clinical staff.
  • Collaborate with physicians, nursing staff, and other healthcare professionals.
  • Provide patient, family, and caregiver education regarding wound and ostomy care.
  • Develop and deliver staff education, orientation, and continuing education programs.
  • Perform comprehensive pressure injury risk assessments and initiate preventive measures.
  • Participate in quality improvement initiatives, prevalence studies, and nursing quality programs.
  • Assist with the selection of wound care supplies, equipment, and technologies.
  • Monitor and manage specialty wound care equipment, including specialty beds and negative pressure wound therapy devices.
  • Maintain patient confidentiality and ensure compliance with organizational policies.
  • Identify and coordinate community resources to support patient care needs.
