About The Position
This position involves traveling to patient homes throughout the assigned service area to provide nursing care. The role requires assessing, planning, implementing, and evaluating patient care plans, administering medications, performing various nursing procedures, and educating patients and families. Collaboration with healthcare professionals and accurate documentation are key aspects of this role.
Requirements
- Minimum 2 years of Med/Surg experience in an acute care hospital or post-acute setting.
- Most recent experience must be within the past year.
- Previous Home Care experience (traveling to patient homes and providing care).
- OASIS documentation experience.
- Active New York State RN License
- Primary Source Verification of RN License
- AHA Basic Life Support (BLS)
- Recent Home Care experience.
- Experience completing OASIS assessments is required.

- EPIC EMR experience
Responsibilities
- Assess, plan, implement, and evaluate patient care plans in collaboration with healthcare professionals.
- Provide direct nursing care in patients' homes.
- Administer medications and monitor patient responses.
- Perform wound care, dressing changes, catheter care, and IV therapy.
- Monitor patient vital signs and report adverse reactions promptly.
- Educate patients and families regarding treatment plans and ongoing care.
- Collaborate with interdisciplinary teams on discharge planning and continuity of care.
- Maintain accurate patient records and clinical documentation.
- Adhere to established nursing standards, best practices, and protocols.
