RN Wound Care Nurse - Home Health - Registered Nurse
About The Position
This role is for an experienced Registered Nurse with a passion for wound care and helping patients heal in the comfort of their homes. The RN Wound Care Nurse will provide specialized wound, skin, and ostomy care throughout The Woodlands, Spring, and surrounding communities. The ideal candidate enjoys working independently, building meaningful relationships with patients, and utilizing clinical expertise to improve patient outcomes and quality of life. Requirements
- Current, unrestricted Texas Registered Nurse (RN) license
- Current BLS/CPR Certification (in-person certification required)
- Negative TB skin test or chest X-ray within the past year
- Minimum one (1) year of documented nursing experience in a home health agency, primary care clinic, wound care clinic, hospital, or healthcare facility preferred
- Strong wound assessment and treatment skills
- Knowledge of evidence-based wound care practices and skin integrity management
- Current and valid Texas Driver's License
- Current automobile insurance
- Reliable transportation
- Strong communication, assessment, and documentation skills

Responsibilities
- Perform comprehensive wound, skin, ostomy, and continence assessments
- Evaluate patient skin condition and wound care needs upon admission and on an ongoing basis
- Develop, implement, and update individualized wound care plans based on clinical findings and physician orders
- Establish measurable goals and recommend appropriate nursing interventions to promote healing and prevent complications
- Perform wound care treatments and dressing changes according to physician orders and evidence-based practices
- Monitor wound healing progress and identify changes requiring intervention
- Educate patients, family members, and caregivers on wound management, skin integrity, infection prevention, and treatment plans
- Collaborate with physicians and other healthcare professionals regarding wound care recommendations and patient progress
- Communicate significant changes in patient condition to the Director of Nursing and other appropriate care team members
- Maintain accurate, detailed, and timely clinical documentation
- Adhere to agency policies, infection control standards, and nursing practice regulations
- Treat patients, families, caregivers, and team members with professionalism, courtesy, and respect
