About The Position

This is a full-time RN position that will provide on-call support as part of our On-Call Team. The On-Call Registered Nurse assists patients and caregivers in the eight county Big Bend Hospice services area through the dying process. This position works a 7 day on, seven days off schedule. FT On call Friday 5pm to Monday 8am for Bay, Washington, Holmes, Jackson, Calhoun, Gulf counties. Willing to provide patient care in adjacent counties as situations warrant.

  • Must have a current Florida Driver’s License, valid auto insurance, and reliable transportation.
  • Valid/current Florida licensure as a RN.
  • Bachelor of Nursing Degree required.
  • 3 years of nursing experience, preferably in hospice or home health.
  • Hospice/Palliative Care experience required.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift 15 pounds at times.
  • Must be able to access and navigate each department at the organization’s facilities.
  • Must possess a valid Florida Drivers' License, current auto insurance, and reliable transportation.

Nice To Haves

  • CHPN preferred.

Responsibilities

  • Supervises LPN and Hospice Aide as needed.
  • Provides quality patient care and manages direct care to patients and families as part of the Interdisciplinary Team [IDT], incorporating appropriate nursing intervention and follow-up. Participates in providing 24-hour/day, 7-day/week patient care. [on-call rotation coverage as needed].
  • Provides palliative nursing care to patients and families applying appropriate nursing interventions which incorporate physical, psychosocial, spiritual, and cultural components consistent with the established patient/family plan of care as developed by the IDT.
  • Revises and updates the patient/family plan of care in response to changing patient and family needs collaborating with other members of the IDT to determine covered services that are consistent with the patient/family goals of care.
  • Completes all clinical documentation consistent and compliant with the state nurse practice act, best practice hospice professional nurse standards, licensing/regulatory guidelines, and organizational policy/procedures at the point of care as appropriate.
  • Communicates effectively with patients, families, physicians, vendors, pharmacies and/or facility staff to enhance the coordination of patient care.
  • Demonstrates clinical skills regarding case management, application of Big Bend Hospice [BBH] policy and procedure, intravenous skills, pain medication titration, symptom management and patient/family relationships.
  • Demonstrates competence in a variety of patient visit types and care situations (on-call visit, death event, continuation of Continuous Home Care, admission, discharge, transfer, triage revocation, General Inpatient).
  • Provides and reinforces family/caregiver education designed to enhance their comfort and independence in providing patient care and support.
  • Performs other duties as requested. Designates Essential Function of the position.
