About The Position

This role involves providing skilled nursing visits to patients in their homes throughout St. Paul and surrounding communities. The position offers independence, allowing nurses to manage their day, conduct clinical assessments, provide care, educate patients and families, and document their findings. The nurse will be responsible for a variety of skilled nursing tasks, including wound care, medication management, and patient education, with the goal of supporting safe transitions home and preventing unnecessary hospital visits. The company emphasizes trusting nurses with clinical responsibility and independence, offering a supportive team environment and established processes.

Requirements

  • Active Minnesota Registered Nurse (RN) license.
  • Previous experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment.
  • Strong clinical assessment and critical-thinking skills.
  • Confidence working independently.
  • Excellent patient and caregiver communication skills.
  • Strong organization and time-management abilities.
  • Ability to recognize and appropriately escalate changes in condition.
  • Reliable transportation and willingness to travel throughout the St. Paul service area.

Responsibilities

  • Provide skilled nursing care to patients throughout the St. Paul territory.
  • Conduct comprehensive RN assessments.
  • Perform skilled home health nursing visits.
  • Provide wound assessment and wound care.
  • Manage medications, including reconciliation and administration of injections.
  • Monitor acute and chronic conditions.
  • Educate patients and caregivers on health conditions and care plans.
  • Identify changes in patient condition and escalate concerns appropriately.
  • Communicate clinical concerns with physicians.
  • Coordinate with physical therapy, occupational therapy, speech-language pathology, social work, and other clinicians.
  • Support safe transitions home following hospitalization or rehabilitation.
  • Help prevent avoidable emergency department visits and rehospitalizations.
  • Complete timely documentation in Homecare Homebase (HCHB).
