About The Position
Home Health Care, Inc. is hiring an RN Visit Nurse to provide skilled nursing visits throughout St. Paul and surrounding communities. This role offers independence and the opportunity to make clinical decisions in patients' homes. The ideal candidate is organized, confident, clinically strong, and self-directed, with the ability to manage their day and provide comprehensive care. The position involves assessing patients, providing skilled nursing care, educating patients and families, and collaborating with a clinical team.
Requirements
- Active Minnesota Registered Nurse (RN) license
- Previous experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment
- Strong clinical assessment and critical-thinking skills
- Confidence working independently
- Excellent patient and caregiver communication
- Strong organization and time-management abilities
- Ability to recognize and appropriately escalate changes in condition
- Reliable transportation and willingness to travel throughout the St. Paul service area

- Home health experience is valuable.
Responsibilities
- Provide skilled nursing care to patients throughout the St. Paul territory.
- Conduct comprehensive RN assessments.
- Perform skilled home health nursing visits.
- Provide wound assessment and wound care.
- Perform medication reconciliation and management.
- Administer injections and other skilled nursing procedures.
- Monitor acute and chronic conditions.
- Educate patients and caregivers.
- Identify changes in patient condition.
- Communicate clinical concerns with physicians.
- Coordinate with PT, OT, SLP, social work, and other clinicians.
- Support safe transitions home following hospitalization or rehabilitation.
- Help prevent avoidable emergency department visits and rehospitalizations.
- Complete timely documentation in Homecare Homebase (HCHB).
