About The Position

The Skilled Nursing Registered Nurse is responsible for the delivery and supervision of patient care targeting the Skilled Nursing Line of Business through the nursing processes of assessment, diagnosis, planning, implementation, evaluation and coordination of care.

Requirements

  • Valid state driver license and reliable transportation with ability to travel to patient homes.
  • CPR certification.
  • Strong clinical assessment and critical thinking skills.
  • Ability to work independently in home-based settings.
  • Effective verbal and written communication skills.
  • Ability to respond appropriately in emergency situations.
  • Current active Registered Nurse license in the state of practice (or compact license where applicable).
  • Individuals receiving company owned (fleet vehicles) may have additional requirements. MVRs will be reviewed during the fleet process.

Nice To Haves

  • BLS Healthcare certification preferred.
  • Two (2) years of Registered Nurse experience in a clinical care setting preferred.
  • One (1) year of pediatric experience preferred.
  • One (1) year of home health experience is preferred.

Responsibilities

  • Own and coordinate care for the skilled nursing cases to include all assessments, recerts, and discharges visits to patients requiring intermittent professional nursing services.
  • Perform Start of Care and Recertification visits for patients.
  • Perform hospital assessment and establish plans of care on new and existing patients.
  • Monitor and supervise the nursing care of patients within assigned territory.
  • Collaborate and communicate with the case manager and direct patient care staff to ensure quality patient care delivery necessary to achieve and exceed individual goals.
  • Provide patient and family education related to diagnosis, treatment, medications, and prevention; collaborate with interdisciplinary team members to ensure coordinated, quality care.
  • Deliver services in compliance with physician orders, state and federal regulations, accreditation standards, and agency policies.
  • Attend all IDT and mandatory meetings, as well as staff meetings, team conferences, and educational in-services.
  • Participate in an on-call rotation as a member of the case management team.
  • Adhere to productivity guidelines for skilled nursing.
  • Always adhere to infection control and safety protocols.
  • Provide care in environments where airborne precautions may be and must be able to wear a fit-tested N95 respirator or higher-level respiratory protection in accordance with OSHA 29 CFR 1910.134.
  • Successfully complete and maintain OSHA-required medical evaluation and respirator fit testing.
  • The ability to wear required respiratory protective equipment and perform all patient care duties while wearing such equipment for extended periods is an essential function of this position.
  • Maintain professional licensure and required certifications.
  • Perform other job-related duties as assigned consistent with business needs.
