About The Position

The RN Transfer Coordinator facilitates emergency and non-emergency patient transfers to Froedtert Hospital, Froedtert Menomonee Falls Hospital, Froedtert West Bend Hospital, and Froedtert Community Hospitals. The coordinator gathers relevant clinical information from the sending provider, including diagnoses, diagnostic test results, current treatment plans, medical and surgical history, and the reason for transfer. In collaboration with accepting physicians and sending providers, the coordinator uses discretion and independent judgment to determine an appropriate level of care and location, transport mode, and type of unit. The role requires knowledge of EMTALA regulations for emergency department transfers and the ability to serve as a resource to stakeholders. The coordinator prioritizes transfers based on patient acuity and clinical information, follows established workflows and protocols, and may participate in difficult or critical situations. Transfer discussions are documented in EPIC. The role collaborates with physicians, nursing, admitting, and hospital patient flow teams to support timely transfers and appropriate patient placement while optimizing hospital occupancy. The coordinator also facilitates consultation requests between outside-facility providers and Froedtert & MCW physicians, participates in conference calls, documents on behalf of providers, and addresses resulting transfer needs. When working shifts between 0000 and 0800, the coordinator is cross-trained to assist with patient phone triage as needed. This is a hybrid, 0.5 FTE position with 20 standard hours per week. Shifts are a mix of 1200–2030 and 1300–2130 on weekdays, with every third weekend from 1300–2130 and holidays as assigned.
