About The Position

The Registered Nurse (RN) Team Coordinator serves as a lead clinical resource within the ambulatory clinic or specialty clinic through the provision of excellent ambulatory nursing care with a patient centered approach to all interactions. The RN Team Coordinator is responsible for providing supervision and leadership to patient care team staff in the Patient Care Services Department. The RN Team Coordinator possesses strong clinical, leadership, education, and advocacy skills. The RN Team Coordinator has responsibility for the day-to-day operations of a patient care team and for working collaboratively with the other team coordinators and Clinic Managers at their respective clinic sites. The RN Team Coordinator functions as a resource person for nursing staff regarding clinic operations issues. Additionally, the RN Team Coordinator functions in various clinical roles, providing telephone triage, patient education, direct patient care, anticipatory guidance, care coordination, and evaluation of nursing staff performance. The incumbent works under the guidance and supervision of the Clinic Manager. Provider staff may delegate activities to a Registered Nurse within the licensing and RN scope of practice within the State of Wisconsin. NOTE: This position is regularly exposed to blood borne pathogens, diseases, and cleaning/sterilization solutions.

Requirements

  • Current licensure as a registered nurse in the State of Wisconsin is required.
  • One (1) to Three (3) years of professional nursing experience is required.
  • Possession of or eligibility for a valid Healthcare Provider CPR/AED certificate is required.
  • Ability to travel to various work sites is required.
  • Employee is expected to provide personal transportation.
  • Possession of a valid, Wisconsin driver license if the employee is operating a motor vehicle is required.
  • Knowledge of scientific nursing principles, methods, and techniques in an ambulatory setting as a basis for quality decision-making ability is required.
  • Current knowledge of and maintenance of nursing trends, techniques and certifications is required.
  • Participation in pertinent continuing education courses is required.
  • Excellent leadership skills are required.
  • Excellent problem-solving and analytical skills are required.
  • Ability to recognize and identify symptoms and problems, with good nursing assessment skills is required.
  • Ability to be resourceful and flexible in emergency situations is required.
  • Ability to review new trends and techniques and update knowledge base is required.
  • Excellent verbal communication and listening skills both in-person and on the phone are required.
  • Excellent assessment skills and the ability to prioritize changing patient conditions is required.
  • Knowledge of interviewing practices and techniques used in order to gather information is required.
  • Excellent customer service and strong interpersonal skills are required.
  • Ability to be culturally sensitive and work with a diverse patient population is required.
  • Knowledge of and/or ability to learn and use computer-based applications such as MS Office, electronic medical record (EMR) systems and scheduling applications is required.
  • Must type a minimum of 30 WPM with minimal errors.
  • Knowledge of medical records and appropriate documentation is required.
  • Basic understanding or ability to learn about reimbursement/insurance practices for medical care is required.
  • Ability to maintain good working relationships with patients and staff is required.
  • Ability to climb, stoop, kneel and crouch is required.
  • Ability to bend, reach, lift, pull, push and/or carry up to 10 pounds regularly and up to 50 pounds occasionally is required.
  • Ability to stand and sit for prolonged periods of time is required.
  • Ability to continuously stand and transport, with occasional sitting is required.

Nice To Haves

  • Bachelor’s degree in Nursing from an accredited college or university is preferred but not required.
  • Experience working in an ambulatory or clinic setting is preferred.
  • If assigned to a specialty area, knowledge, and skills to provide patient care appropriate to the specialty is desirable.
  • A minimum of one (1) year of RN experience at Group Health Cooperative is strongly preferred.
  • Prior leadership or supervisory experience is desirable.
  • Experience with Epic Care EMR is desirable.

Responsibilities

  • Providing supervision and leadership to patient care team staff in the Patient Care Services Department.
  • Managing the day-to-day operations of a patient care team.
  • Collaborating with other team coordinators and Clinic Managers.
  • Functioning as a resource person for nursing staff regarding clinic operations issues.
  • Providing telephone triage, patient education, direct patient care, anticipatory guidance, care coordination, and evaluation of nursing staff performance.
  • Performing a variety of clinical skills which may include but are not limited to IV Therapy, injections, allergy shots and wound care.
  • Assisting in procedures performed in clinics.
  • Educating patients and/or families as to the nature of disease and providing instruction on proper care, treatments and follow-up.
  • Providing individual and small group problem-solving and guidance.
  • Providing positive guidance, proactive coaching, and timely discipline in the day-to-day supervision of staff.
  • Developing and monitoring the professionalism, skill and teamwork of the assigned Patient Care Services staff.
  • Maintaining quality, safety and/or infection control standards.
  • Maintaining patient confidentiality.
  • Working as a member of a team and maintaining good working relationships with staff and patients.
  • Maintaining good organizational skills in the delivery of clinical care.
  • Maintaining a professional demeanor at all times.
