About The Position

Focused Post Acute Care Partners is seeking an RN Supervisor for Focused Care at Humble in Humble, Texas. Under the direction of the Director of Nursing, the RN Supervisor assists in supervising nursing services and setting standards of care. The role supports a professional, skilled program of care and treatment that addresses each resident’s medical, physical, emotional, and psychological needs. This position works with the Director of Nursing, physicians, team members, residents, and the interdisciplinary team to support clinical outcomes and continuity of care. Work hours may be flexible depending on facility needs. The schedule is assigned and may include weekends, evenings, and holidays. This is a non-exempt position.

Requirements

Be a registered nurse licensed and in good standing with the Texas State Board of Nurse Examiners in Texas.
Be able to safely push, pull, lift, bend, kneel, and reach, including lifting up to 50 pounds, with or without reasonable accommodations.

Responsibilities

Support the Director of Nursing in monitoring, evaluating, and organizing direct nursing care.
Communicate effectively with the Director of Nursing, physicians, team members, and residents to promote clinical outcomes and continuity of care.
Work cooperatively with the interdisciplinary team to assess, develop, implement, and evaluate resident care plans.
Make frequent rounds throughout the shift and assist residents and staff when necessary.
Use critical thinking to respond to resident conditions and implement interventions to help prevent avoidable rehospitalizations.
Monitor completion of electronic medical records and documentation.
Educate staff on policies and processes when necessary.
Support newly hired nursing staff.
