Requirements

  • High School diploma or equivalent
  • Registered Nurse with active unrestricted license in the State of Nevada and a graduate of an approved school of professional nursing
  • Current CPR certification (or obtain within 30 days of hire)
  • 2+ years of experience in Home Health patient care setting, providing direct patient care
  • 1+ years of performance management with supervisory experience
  • Experience working with an electronic medical record application
  • Demonstrates thorough knowledge of current nursing practice, professional standards of care, and state & federal regulations regarding home care
  • Basic PC skills to include Windows, Outlook or other email system
  • High level of critical thinking skills to analyze multiple sources of information to formulate logical conclusions and actions
  • Proven ability to plan, organize, prioritize and direct the work of others and communicate objectives clearly
  • Proven excellent observation skills, verbal & written communication skills, attention to detail and demonstrated competence in core leadership skills
  • Demonstrated ability to successfully utilize clinical information systems for operation and performance management functions
  • Valid Nevada driver's license and maintain personal auto insurance coverage

Responsibilities

  • Oversees a caseload of 100-175 home care patients and provides direct supervision to clinical staff (RN, LPN, MSW and HHA) assigned to the team.
  • Ensures home care services are compliant with state and federal regulations regarding home care services and nursing practice.
  • Ensures staff is compliant with Agency policies and standards, state and federal regulations through ongoing performance evaluation and management including, but not limited to, annual supervisor visits, routine case conferences, and routine 1:1 conferences.
  • Serves as mentor and role model for staff by providing clinical and supervisory guidance and support.
  • Ensures patient care is coordinated and documented effectively amongst disciplines through clinical record review and multidisciplinary case conferences.
  • Assists with management of complex patient/caregiver situation by providing consultation to field staff to include home visits.
  • Ensures patient care provided by team is coordinated to include staffing for day, evening, weekends, holidays and that team members participate in Agency on-call coverage.
  • Conducts clinical field supervision and performance management monitoring of attendance, productivity, overtime, supplies and cost per visit to ensure compliance with Agency standards.
  • Utilizes management reports and other resources to ensure ongoing management of patient outcomes and Team/Agency operational performance.
  • Assumes individual responsibility for professional competence and professional nursing practice.
  • Provides oversight of the plans of care for patients assigned to Team to include monitoring of complex care plans, documentation, and regulatory compliance review; utilizes key reports and clinical record review to ensure ongoing oversight of these key issues.
  • Coordinates with RN Clinical Review Specialist regarding performance improvement activities including day-to-day operational issues and Team/Division/Agency initiatives.
  • Interviewing, selection and retention of qualified staff for the Team.
  • Participates actively in the orientation of new organizational personnel.
  • Assists in the formulation, revision, implementation and evaluation of policies and procedures as well as strategic goals and objectives.
  • Promotes positive customer service at all times both internally and externally in interactions with patients, families, referral sources, vendors, and co-workers of the team and other Teams/Divisions of Southwest Medical Home Health.
  • Team staff compliance with HR requirements (licensure, CPR, TB, policy and procedure review, competencies, etc.).
  • Assists in the formulation, revision, implementation and evaluation of in-service and continuing education programs.
  • Performs other duties as assigned by the Associate Director of Nursing.
