About The Position

The nursing role is grounded in Client and Family Centred Care, focusing on providing and coordinating home health nursing services according to professional standards and VHA policies. We are seeking two Registered Nurses (RNs) or Registered Practical Nurses (RPNs) for shift nursing opportunities in the Toronto Central region, one for the morning shift and one for the evening shift.

Requirements

  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • RPN diploma or RN diploma/degree from a recognized university or college.
  • Hold a valid First Aid or CPR Certification.
  • Demonstrate effective communication (written and verbal) and documentation skills.
  • Demonstrate reliable attendance, punctuality, and availability for the role.
  • Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.

Nice To Haves

  • Membership of your Professional Association preferred (RNAO/WeRPN).
  • A moderate level of wound care or venipuncture knowledge and experience is an asset.
  • Acute Care and Medical-Surgical nursing experience is an asset.
  • Knowledge of community nursing is an asset.
  • A valid Ontario driver’s license (G License) and a reliable vehicle for traveling is an asset.

Responsibilities

  • Assesses, monitors, and manages care planning and care coordination.
  • Evaluates care plan interventions through on-going reassessment and evaluation results.
  • Ensures the quality of care of patients by identifying and acting on factors which affects practice autonomy and delivery of quality care.
  • Demonstrates professionalism, leadership, judgement, and accountability in interactions with client and families.
  • Supports client safety care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
  • Informs nursing supervisor of any client activities and relevant concerns or changes in the client or the client care plan and informs/submits appropriate documentation to the case coordinator.
  • Maintains documentation standards according to the College of Nurses of Ontario.
