About The Position

This specific role is for a full-time Registered Practical Nurse (RPN) or Registered Nurse (RN) to support the Child & Family Program in the Toronto Central region. The program provides individualized, one-on-one care to clients from newborns up to 25 years of age with complex medical needs. As a visiting nurse, the role involves delivering home-based nursing care, supporting both the health and overall well-being of children within a family-centered environment. The program also emphasizes continuous professional development through educational opportunities, including access to the SickKids Connected Care program.

Requirements

  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • RPN diploma or RN diploma/degree from a recognized university or college.
  • Hold a valid First Aid or CPR Certification.
  • Demonstrate effective communication (written and verbal) and documentation skills.
  • Demonstrate reliable attendance, punctuality, and availability for the role.
  • Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
  • Experience providing tracheostomy care, ventilator care, and in G-tube feeding.
  • Valid Ontario driver's license (G2 or G) and access to a reliable vehicle.

Nice To Haves

  • Membership of your Professional Association preferred (RNAO/WeRPN).
  • A moderate level of wound care or venipuncture knowledge and experience is an asset.
  • Acute Care and Medical-Surgical nursing experience is an asset.
  • Knowledge of community nursing is an asset.
  • A valid Ontario driver’s license (G License) and a reliable vehicle for traveling is an asset.

Responsibilities

  • Provide and coordinate home health nursing services to diverse groups of individuals, families, and groups.
  • Deliver home-based nursing care to children with complex medical needs, supporting their health and overall well-being.
  • Perform seizure management, medication administration and monitoring, palliative care, G-tube feeding, mechanical ventilator care, and tracheostomy care.
  • Conduct ongoing assessments and provide assistance with personal care such as diaper changes and hygiene needs.
  • Assess, monitor, and make care planning and care coordination.
  • Evaluate care plan interventions through on-going reassessment and evaluation results.
  • Ensure the quality of patient care by identifying and acting on factors that affect practice autonomy and delivery of quality care.
  • Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families.
  • Support safe client care through the development of knowledge and skills in accordance with principles of continuous quality improvement.
  • Inform the nursing supervisor of any client activities, relevant concerns, or changes in the client’s condition or care plan, and submit appropriate documentation to the case coordinator.
  • Maintain documentation standards according to the College of Nurses of Ontario.
  • Participate in a standby/on-call rotation to be available for answering calls and respond, if needed.
  • Participate in team meetings, as required during the stated availability.
