About The Position

This role involves monitoring patient vital signs and health data using remote devices, analyzing trends to identify potential health risks, and providing patient education and support. The nurse will collaborate with healthcare professionals to tailor care plans, document interactions in electronic health records, and coordinate patient care with various teams. The position requires home visits for equipment setup, education, and troubleshooting, as well as monitoring RPM devices across multiple service lines via a partner platform. The nurse will also be responsible for accurate documentation of RPM care management time for compliance and billing purposes.

Requirements

  • Graduate of an accredited school of nursing
  • One year RPM related experience in a healthcare setting, OB and chronic disease management preferred
  • Communicates and interacts professionally with others
  • Works cooperatively with patients and their significant others, all clinic staff, other hospital workers, director, visitors, and contracted staff
  • Works as part of a team and independently
  • Provides health certification at the time of employment, indicating that employee is free of any infectious process, including tuberculosis, and is in general good health. Certification must be renewed annually
  • Permits criminal background check as required by Indiana State Department of Health
  • Provides proof of valid driver’s license
  • Registered Nurse, Licensed Practical Nurse - currently licensed by the State of Indiana
  • CPR (BLS) certified – to be renewed as required
  • Must be self-motivated

Responsibilities

  • Monitor patient vital signs and health data using remote devices
  • Analyze data trends to identify potential health risks
  • Provide patient education and support about health data
  • Collaborate with healthcare professionals to tailor care plans
  • Document patient interactions accurately in electronic health records – daily outreach calls to patients
  • Coordinates patient care with care management/PNN team members, specialists and primary care providers
  • Works independently as needed with a high degree of detail orientation
  • Home visits required
  • Provides documentation (to the patient’s primary care provider or specialist) that clearly reflects the care being provided – document RPM data in EHR
  • Accurately track all patient RPM care management time to ensure compliance with programming/billing requirements
  • Monitor all RPM devices; multi-service line (not limited to: HTN, Diabetes, Heart Failure, COPD, Maternal Health) via partner platform
  • Answers phone with a smile and directs calls to the appropriate team member
  • “Manage up” all staff members and provide a professional, warm handoff as appropriate
  • Communication with clinical/clerical team
