About The Position
Phoenix Hospice is hiring RN Case Managers to provide skilled, compassionate hospice nursing care to patients and families in Springfield, Missouri, and surrounding areas. The role supports patients and families through end-of-life care, helping them understand the hospice process, make healthcare decisions, and navigate goals-of-care discussions with clarity and dignity. The nurse works with an interdisciplinary hospice team and coordinates care to address patient and family needs. Full-time and PRN availability are offered.
Requirements
- Active Missouri or Compact RN license.
- Valid driver's license and auto liability insurance.
- Strong assessment, communication, and care coordination skills.
Nice To Haves
- Hospice or long-term care nursing experience is preferred.
Responsibilities
- Provide compassionate hospice nursing care to patients and families.
- Educate patients and caregivers about the hospice process and plan of care.
- Guide families through goals-of-care discussions and healthcare decisions.
- Conduct initial and ongoing comprehensive assessments.
- Perform hospice evaluations and informational visits.
- Collaborate with physicians, Hospice Medical Directors, and interdisciplinary team members.
- Coordinate care to ensure patient and family needs are met.
- Communicate changes in patient condition and update plans of care as needed.
