About The Position

Phoenix Home Care & Hospice is seeking a hospice RN Case Manager to provide compassionate nursing care to clients and families dealing with a terminal diagnosis. The nurse works with an interdisciplinary team to develop and implement a patient- and family-centered plan of care, support discussions about goals of care, and guide healthcare decisions to promote quality of life. The position serves Jefferson City, Missouri, and surrounding areas, with a Monday–Friday schedule from 8:00 a.m. to 5:00 p.m. The stated pay range is $65,000–$90,000.

Requirements

• Missouri or Compact RN license.
• Hospice or long-term nursing experience.
• Valid driver's license and auto liability insurance.

Responsibilities

• Educate patients and family members about the process and plan of care, and guide them in healthcare decisions.
• Consult with the attending physician and Hospice Medical Director about changes in a patient's condition that require order or care-plan changes.
• Coordinate hospice care with other team members and patients and family members to ensure needs are met.
• Conduct initial and ongoing comprehensive assessments.
• Evaluate patients for hospice care and conduct informational visits.
