About The Position

The RN Program Coordinator provides comprehensive care coordination for patients with complex conditions across the continuum of care. This role serves as a key liaison among providers, patients, families, laboratories, pharmacies, and external agencies to ensure timely diagnosis, treatment, and long-term management. The coordinator also supports clinical operations, research initiatives, and patient advocacy within a multidisciplinary and academic care environment.

Requirements

  • Bachelor's Degree in Nursing
  • 3 years of nursing experience
  • (RN) REGISTERED NURSE Licensure by Board of Nurse Examiners for the State of Texas Upon Hire
  • (BLS) BASIC LIFE SUPPORT Course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire

Responsibilities

  • Coordinates intake, evaluation, and longitudinal care for patients with complex conditions.
  • Serves as the primary nursing liaison among multidisciplinary teams and external providers.
  • Reviews referrals and ensures appropriate documentation and follow-up.
  • Coordinates clinical testing, including prior authorizations, specimen collection, lab communication, and result tracking.
  • Manages patient communication through calls, MyChart messages, and nurse visits.
  • Monitors surveillance labs, communicates results to patients and providers, and maintains patient tracking databases.
  • Facilitates transitions of care, including pediatric-to-adult clinic transitions and coordination with inpatient and outpatient services.
  • Coordinates specialty clinic visits and multidisciplinary care plans.
  • Obtains and integrates external medical records into the patient chart.
  • Coordinates complex therapies, infusion services, and/or specialty medication management.
  • Obtains prior authorizations for infusions, program-specific clinical procedures and treatments, and/or high-cost specialty medications.
  • Communicates with pharmacies, infusion centers, home health agencies, and other ancillary services to ensure continuity of care.
  • Assists with durable medical equipment orders and home health service coordination.
  • Monitors, documents, and reports adverse reactions as needed.
  • Provides education on disease processes, treatment plans, and long-term management.
  • Serves as a consistent point of contact for patients and families navigating complex care.
  • Assists patients in accessing financial assistance programs, including foundation, pharmaceutical, and government resources.
  • Supports applications for Medicaid (Title XIX), newborn screening benefits, and other state/federal programs.
  • Advocates for patients throughout enrollment and assistance processes.
  • Maintains a database of program patients and track care metrics, including follow-up visits and lab compliance.
  • Coordinates care with state health departments (e.g., Texas and Oklahoma) for program patients and newborn screening programs.
  • Completes patient screening applications and ensure compliance with state requirements.
  • Supports workflow development, program growth, and process improvement initiatives.
  • Participates in multidisciplinary meetings, case reviews, and program development efforts.
  • Coordinates lab specimen collection for research and testing.
  • Supports clinical research activities, including research medication administration.
  • Enters and tracks metabolic lab results and communicate findings to providers.
  • Participates in academic initiatives, including conferences and educational activities.
  • Assists with clinic and inpatient coordination as needed.
  • Supports onboarding and training of new staff.
  • Performs other duties as assigned to support departmental and program goals.
