About The Position
Suncoast Hospice, part of Empath Health, is seeking a caring and motivated RN Case Manager to provide care in Pinellas County. The position covers both home and facility teams. Suncoast Hospice provides compassionate end-of-life care and supports patients living with advanced illness, along with their families and caregivers.
Requirements
- Graduate of an approved nursing program.
- Current Florida RN license.
- At least one year of experience providing direct patient care in an acute-care hospital environment.
Nice To Haves
- Previous experience as a hospice RN.
Responsibilities
- Review and update care plans, physician orders, pharmacy profiles, and other records.
- Educate patients, family members, and other caregivers about the disease process and decline, available palliative interventions, the dying process, and safety practices.
