The RN Patient Navigator - Neurosurgery is responsible for the ongoing evaluation and quality improvement of the Navigator Program. This role ensures quality patient care by adhering to the policies of WFBMC, involving patients and their families in identifying their needs, creating a plan to meet those needs, and effectively communicating the plan across the continuum of care. The navigator identifies barriers to efficient and effective patient care management and seeks strategies to remove them. They will implement outreach efforts to establish and maintain positive working relationships with key community agencies, collect and maintain accurate program data, and deliver reports demonstrating outcomes and performance improvement activities. This position requires training in motivational interviewing and health coaching for chronic care management, and active participation in shared governance for professional growth. The role also involves providing age/developmentally appropriate patient care.
