About The Position

This RN position will serve to support both existing Triage and Case Manager functions. Triage-Serves as the first clinical contact for a patient seeking advice or treatment for an acute condition. The Triage Nurse provides health advice to patients over the phone while performing an evaluation of patient symptoms and provides recommendations for escalating treatment. The Triage Nurse follows established procedures and protocols, and documents calls as required to ensure the safe, effective and appropriate disposition of health related problems. Case Management-Actively participates with patients to identify and facilitate options and services for meeting their individual's health needs, with the goal of decreasing fragmentation and duplication of care, and enhancing quality and cost-effective clinical outcomes. RN Case Manager will develop, implement, and evaluate individualized patient care plans with the healthcare team, and serve as a liaison for working with patients and their families.

Requirements

  • Graduation from an accredited School of Nursing.
  • Minimum of 3 years of clinical med surg experience required.
  • Licensed and a currently a registered nurse in the State of Connecticut.
  • Basic Cardiac Life Support (BCLS) training must be obtained prior to employment or within one (1) month of hire.
  • Excellent communication, critical thinking, and problem-solving skills in using the nursing process to assist patients.
  • Minimum of two years of RN experience required.

Nice To Haves

  • BSN or higher degree preferred.
  • If non BSN, must show evidence of active enrollment in minimum of a BSN program.
  • Community health center/outpatient setting experience preferred.
  • Phone triage experience preferred.
  • Experience using an EMR preferred.
  • Phone Triage experience desired.

Responsibilities

  • Answers and manages nurse-directed patient calls.
  • Coordinates same-day and walk-in appointments when deemed appropriate.
  • Works with the entire health care team, including collaboration with providers, social work and patient navigators.
