About The Position

Family First Homecare (FFHC) is seeking a compassionate, confident, and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide high-quality, one-on-one in-home nursing care for a pediatric patient. This is a meaningful opportunity to support a 17-year-old female patient in the comfort of her home. The patient is receiving both hospice care and in-home nursing services, and the family is looking for an experienced nurse who is comfortable providing comprehensive care independently in the home setting. The family is supportive and involved, and the patient is described as having a supportive home environment. However, the home can be busy, so the ideal nurse should be confident, self-sufficient, and comfortable managing all aspects of care before entering the home. This case is best suited for a nurse who has strong clinical judgment, can remain calm in an active environment, and is comfortable supporting a pediatric patient with higher-acuity needs.

Requirements

  • Active and unencumbered LPN/LVN or RN license in the state of employment, or compact license where recognized
  • Minimum of 1 year of demonstrated nursing experience
  • Current CPR certification with in-person, instructor-led hands-on skills check
  • Working knowledge of Nurse Practice Acts
  • Clear background screening
  • Valid driver’s license and proof of insurance, where applicable
  • Clear TB screening, where applicable
  • Clear Hep B screening, where applicable
  • Ability to read, write, and communicate effectively in English

Responsibilities

  • Provide care within the scope of your nursing license and certification
  • Maintain accurate and timely clinical documentation
  • Evaluate the effectiveness of nursing care on an ongoing basis
  • Communicate changes in patient condition or care needs to the nurse supervisor
  • Consult with physicians regarding changes to the patient’s care plan, as needed
  • Educate patients and caregivers on care-related topics
  • Maintain patient and family confidentiality at all times
  • Follow Family First Homecare policies, procedures, compliance standards, and ethics expectations
  • Participate in required trainings, in-services, and quality improvement initiatives
  • Maintain professional, supportive, and responsive communication with families and team members
  • Mentor or support new team members during patient-specific orientations, as needed
  • Perform other duties as assigned
