About The Position

With 2 floors and 164 beds, this facility provides care for long-term care residents and post-acute rehab patients. It features a long-term care unit and a rehab unit, with each floor having designated units focusing on patient specialties such as general rehab, neuro rehab, traumatic brain injuries, and cancer. The facility offers integrated services with a mission to provide goal-oriented, compassionate care for residents to live healthy and active lives to the greatest level possible. The facility is located on a 10-acre campus with indoor and outdoor spaces designed to improve independence, strength, and confidence. Two main floor courtyards offer outdoor space for rest and renewal. Most patient rooms are private with an attached bathroom and shower, television with cable, phone, and a large window. Each unit has a large and accessible living and dining space, and a menu-based meal service is provided.

Requirements

• Associates in Nursing
• State of MI Registered Nurse (RN)
• AHA or ARC Basic Life Support (BLS) within 90 days
• High School Diploma or equivalent
• Completion of an approved nursing program
• State of MI Licensed Practical Nurse (LPN) upon hire
• AHA or ARC Basic Life Support (BLS) within 90 days

Nice To Haves

• Graduate of an approved nursing program

Responsibilities

• Delivers nursing care responsibly and accountably as described professionally by the American Nurses Association (ANA) and legally by the State of Michigan.
• Demonstrates competency through the critical thinking model known as the nursing process. This includes assessment, diagnosis, outcomes identification, planning, implementation, and evaluation.
• Upholds the standards of professional performance as described by the ANA. This includes ethical practice, culturally congruent practice, communication, collaboration, leadership, education, evidence-based practice and research, quality of practice, professional practice evaluation, resource utilization and environmental health.
• Strives to achieve optimal outcomes.
• Under the direction of the RN/Mid-level Provider/Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care.
• Informs patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN/Mid-level Provider/Physician.
• Documents care provided via computer/written patient record in a manner consistent with guidelines.
