About The Position

Deliver compassionate bedside care to long-term care residents under the guidance of clinical leaders, ensuring dignity, comfort, and safety in their daily lives. Champion resident safety by proactively identifying environmental hazards or emerging emergency situations and swiftly taking corrective action to protect our community. Administer medications and treatments accurately while vigilantly monitoring residents for adverse reactions, directly safeguarding their health and recovery journeys. Contribute to comprehensive health assessments by gathering vital clinical data and performing routine tests, partnering with Registered Nurses to build precise, high-quality care plans. Empower patients and families through clear education on treatment plans, holistic health practices, and post-discharge self-care to ensure a seamless transition and lasting wellness.

Requirements

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Licensed Practical/Vocational Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
  • High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
  • Required professional licensure/certification can be used in lieu of education or experience, if applicable.
