The RN Oncology Navigator (PONN) delivers cancer care across the continuum, from diagnosis through treatment, and implements the transition to survivorship, and end-of-life care. Using the nursing process, the PONN provides patient and family insight with appropriate education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The role involves critical thinking and clinical assessment of patients, providing support and education, managing the complexity of cancer diagnosis, and collaborating with multidisciplinary team members. The PONN also reviews distress screenings for referrals and reinforces education regarding diagnosis, connecting patients to appropriate resources. Additionally, the role involves developing and reporting outcome metrics to drive program development and assessing activities that support standards related to Children's Oncology Group (COG) and community needs assessments. The PONN provides emotional support, advocates for the patient, empowers the patient to self-advocate, and maintains their own knowledge, abilities, skills, and competencies, while role-modeling organizational behaviors of excellence.
