About The Position

The RN Oncology Navigator (PONN) delivers cancer care across the continuum, from diagnosis through treatment. They implement the transition to survivorship, and end-of-life care. Using the nursing process, the PONN provides patient and family insight with appropriate education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The role involves critical thinking and clinical assessment of patients, providing support and education, and managing the complexity of cancer diagnoses. The PONN communicates and collaborates with multidisciplinary team members, reviews distress screenings for referrals, and communicates with the treating physician. They also receive and respond to new patient referrals, reinforce education regarding diagnosis, connect patients to appropriate resources, and develop and report outcome metrics to drive program development. Additionally, the PONN assesses and implements activities that support standards related to Children's Oncology Group (COG) and reviews/strategizes activities that support community needs assessment of site-specific results. The role also includes providing emotional support for patients, advocating on behalf of the patient, empowering the patient to self-advocate, and maintaining their own knowledge, abilities, skills, and competencies while role-modeling organizational behaviors of excellence.

Requirements

  • BSN required
  • Typically requires 5+ years experience in clinical specialty (Oncology)
  • Current RN license
  • CPR certification
  • OCN Certification or Nurse Navigator Certification obtained within 2 years of hire.

  • This position needs to recognize needs and behaviors of specific age groups of patients treated
  • Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
