Be a leader in providing comprehensive preventative cancer care for patients. Assess patients’ barriers to care, psychosocial, and learning needs to advocate for and support patients and caregivers, collaboratively with teams and support services, as we strive to provide equitable care and outcomes for our patients. Facilitate timely and coordinated transitions of care, provide anticipatory guidance, reinforce patient and caregiver education, provide guidance in the shared decision-making process, and provide mental and emotional support to patients and caregivers through the continuum of cancer care. Participate in continuous quality and process improvement initiatives.
