About The Position

The Patient Navigator-RN Cancer is a skilled oncology nurse and specializes in caring for patients eighteen years of age or older with suspected or confirmed malignancies or a hematologic disorder in the outpatient Cancer Clinic. Utilizes discretion and independent judgment in applying the nursing process to the provision of age-specific and developmentally appropriate quality patient care. Case manages and coordinates the continuum of care from referral through the treatment process in collaboration with the physician and the multidisciplinary team. Proactively manages daily clinic activities to ensure efficiency and timely patient visits. Implements research protocols in collaboration with the research team. Facilitates the empowerment of the nursing profession and embraces new knowledge, innovations and improvements in practice to enhance the patient experience and clinical outcomes. Performs other duties as needed.

Requirements

  • Bachelor of Science Degree in Nursing required.
  • Minimum one year of nursing experience required.
  • Current California Registered Nurse (RN) license required.
  • Basic Life Support certification issued by the American Heart Association required.
  • Able to speak, read and write legibly in English.
  • Able to use computer, printer, and software programs necessary to the position, e.g., Word, Excel, Outlook, PowerPoint, electronic medical record, electronic event reporting program.
  • Able to troubleshoot and calibrate patient care equipment.
  • Able to perform technical patient care activities.
  • Able to relate and communicate positively, effectively, and professionally with others.
  • Able to be assertive and consistent in following and/or enforcing policies.
  • Able to work calmly and respond courteously when under pressure.

Nice To Haves

  • Minimum one year of oncology nursing preferred.
  • One year case management or transitional care management experience preferred.
  • Spanish preferred.
  • Oncology Certification Nurse (OCN) preferred.
