About The Position
The RN Nurse Preceptor is a qualified clinical professional who directs, supervises, evaluates, and provides nursing services to patients in the home, as prescribed by the attending physician. The role is recognized as an experienced nurse whose proficiency in home health is evident to peers, patients, and referral sources. In addition to the essential functions of an RN Case Manager, the RN Nurse Preceptor provides clinical orientation, training, and mentoring to new employees or current employees who need assistance. The position is primarily based outside an office and may involve exposure to outdoor conditions. It is classified as medium work, involving sitting and standing and occasionally exerting up to 50 pounds of force.
Requirements
- Associate's degree in Nursing.
- Current, valid RN license issued by the New York State Department of Education.
- At least one year of home care experience.
- Knowledge of state and federal regulations and payer guidelines.
- Current, unexpired New York State driver's license.
- Current CPR certification.
- Critical thinking skills.
- Strong organizational and time-management skills, including the ability to multitask.
- Strong documentation skills.
- Clinical competency in nursing skills.
- Effective communication and collaboration with a multidisciplinary care team.
Nice To Haves
- BSN degree preferred.
Responsibilities
- Provide in-home clinical orientation through one-to-one teaching and modeling of job duties during co-visits.
- Deliver classroom training as needed.
- Evaluate and document preceptee skill performance to determine competency.
- Coordinate with new clinical employees throughout orientation, providing mentoring, training, and advocacy.
- Participate in weekly case conferences or staff meetings with the preceptee and manager during the orientation period.
- Perform other duties as assigned.
- Direct, supervise, evaluate, and provide home nursing services to patients as prescribed by the attending physician.
