About The Position

Interim HealthCare is seeking a Home Health Registered Nurse to provide patient-centered care in patients’ homes. The role offers flexible scheduling and autonomy, with nurses able to set a schedule and choose how much they work. Care is often provided after a patient is discharged from a hospital, rehabilitation center, or nursing facility.

Requirements

• Graduate of an accredited nursing program.
• Active RN license in Pennsylvania.
• At least 1 year of nursing experience.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in the nursing specialty.
• Strong communication skills and compassion.

Responsibilities

• Provide home-based care to patients with illnesses, injuries, and chronic diseases.
• Work with physicians and oversee LPNs, CNAs, and aides involved in patient care.
• Examine patients, conduct assessments, document progress, and report changes to physicians.
• Educate patients about their plan of care, prescribed medications, therapy, diet, and exercise.
• Coach family members on the patient’s plan of care, medications, and home safety.
• Ensure care goals are met and coordinate discharge from services.
