RN Nurse Consultant Social Adult Day Care
About The Position
Grace Community Care & Homes is developing its Adult Day Programs department and seeks a RN Nurse Consultant Social Adult Day Care. The role will contribute to person-centered services, reliable operations and accurate documentation. This is a planned role. Start date and duties are contingent on applicable program approvals, funding, credentialing and staffing readiness. Services may begin only within the approved scope. Location and schedule New Jersey, with administrative coordination through Grace and on-site or community assignments as applicable. This is a part-time role. The final assignment and weekly schedule will be discussed during hiring. Clinical services begin only at approved locations and within the program's authorized scope.
Requirements
- Active NJ RN license
- Relevant community or adult-care experience
- Capacity to provide the consultation required by the final program
- Relevant credentials, experience, availability and a resume
Responsibilities
- Consult on health profiles, observations, staff training, escalation procedures and care planning for the social day program.
- Work collaboratively with department leadership, intake, credentialing, billing and quality teams.
- Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope.
- Participate in orientation, competency review and quality improvement.
