About The Position
Grace Community Care & Homes is developing its Adult Day Programs department and seeks a Social Worker Adult Day Programs. The role will contribute to person-centered services, reliable operations and accurate documentation. This is a planned role. Start date and duties are contingent on applicable program approvals, funding, credentialing and staffing readiness. Services may begin only within the approved scope. Clinical services begin only at approved locations and within authorized scope.
Requirements
- NJ social-work credential appropriate to assigned scope
- Adult or geriatric social-service experience
- Required supervision
- Relevant credentials, experience, availability and a resume
Responsibilities
- Assess social needs, support participant and family care planning, coordinate community resources and maintain social-service documentation.
- Work collaboratively with department leadership, intake, credentialing, billing and quality teams.
- Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope.
- Participate in orientation, competency review and quality improvement.
