About The Position

The RN Navigator works as part of a multidisciplinary team, serving as an advocate, educator, and communicator for oncology patients and their families. The role coordinates patients through the continuum of care, connects disciplines with supportive and community services, supports follow-up care, and contributes to treatment outcome tracking and process improvement. The position is based at UofL Health – Brown Cancer Center – UMC in Louisville, Kentucky, and may require travel to multiple locations within the UofL Health system.

Requirements

• Graduate from an accredited or approved school of nursing.
• Associate Nursing Degree required; BSN required within two years of hire.
• Current RN license in Kentucky.
• At least two years of bedside oncology experience.
• Excellent communication skills and ability to interact with nurses, physicians, and skilled healthcare professionals.
• Basic Life Support (BLS) certification from the American Heart Association.
• Ability to work autonomously without direct supervision.

Nice To Haves

• Oncology Certified Nurse (OCN) certification is recommended or must be obtained within two years of employment.
• Academy of Oncology Nurse and Patient Navigator certification is recommended or must be obtained within two years of employment.
• Previous Cerner experience.

Responsibilities

• Educate patients and families about disease processes, treatment options, and potential side effects.
• Coordinate patients through the continuum of care in collaboration with the multidisciplinary team.
• Serve as a liaison between disciplines and supportive or community services.
• Coordinate weekly multidisciplinary conferences and database tracking analysis for treatment outcome purposes.
• Compile data, track outcomes, and make recommendations for process improvement.
• Provide referrals and support for patients’ follow-up care.
• Travel to multiple locations within the UofL Health system as needed.
