About The Position

This position is for an RN Navigator in the Pediatric Complex Care Clinic. The role involves caring for pediatric patients with multiple chronic health conditions, acute episodes of care, and behavioral health challenges, while promoting prevention and wellness. The nurse will provide evidence-based, high-quality nursing care via telephone and MyChart, assist with care coordination and clinic procedures, and collaborate with a multidisciplinary team to achieve positive patient outcomes. Education and guidance will be provided to patients and families, utilizing evidence and science to guide practice.

Requirements

Bachelor's Degree in Nursing (BSN) Required
3 years of relevant clinical nursing experience Required
Registered Nurse license in the state of Wisconsin Upon Hire Required
Basic Life Support/CPR Upon Hire Required

Nice To Haves

Master's Degree in Nursing, Social Work, Public Health, Healthcare Administration, or related field Preferred
1 year of relevant experience in the specific patient population being supported Preferred
Applicable Oncology certification based on patient population Preferred
